Understanding Different Brains, Mental Health and the Importance of the Right Support

Neurodivergence is becoming increasingly recognised, giving many people language for experiences they may have struggled to understand for much of their lives.

For some people, discovering they are autistic or have ADHD can bring an enormous sense of recognition. Experiences previously described as being “too sensitive,” “difficult,” “lazy,” “disorganised,” or “too much” can suddenly be understood through a completely different lens.

But neurodivergence and mental health are complex.

Autism Spectrum Disorder (ASD) and Attention-Deficit/Hyperactivity Disorder (ADHD) are neurodevelopmental conditions. When someone is both autistic and has ADHD, the informal term AuDHD is commonly used.

Conditions such as Post-Traumatic Stress Disorder (PTSD), Obsessive-Compulsive Disorder (OCD), and Borderline Personality Disorder (BPD) are different clinical conditions. However, they can coexist with autism or ADHD, and certain characteristics may overlap.

Understanding these differences matters because similar-looking behaviours can have very different underlying causes.

The aim should never simply be to find a label. It should be to understand the person behind the symptoms.

What Is Neurodivergence?

Neurodivergence is a broad term used to describe people whose brains process, learn, communicate, regulate attention, or experience the world differently from what is considered neurologically typical.

Neurodiversity recognises that variation between human brains naturally exists.

Being neurodivergent can involve challenges, particularly when environments are built around neurotypical expectations. It can also involve individual strengths, perspectives, interests, and ways of thinking.

Understanding neurodivergence means moving away from:

“What is wrong with this person?”

and towards:

“How does this person experience the world, and what support do they need?”

Autism Spectrum Disorder (ASD)

Autism Spectrum Disorder (ASD) is a lifelong neurodevelopmental condition that affects how a person experiences, processes, and interacts with the world.

Every autistic person is different, which is why autism is described as a spectrum.

Autistic people may experience differences involving:

Social communication and interaction
Sensory processing
Preference for routine and predictability
Repetitive movements or behaviours
Highly focused interests
Emotional processing
Understanding or navigating social expectations
Sensory or social overwhelm

Autism does not always look like common stereotypes.

Some autistic people, particularly women and people diagnosed later in adulthood, become highly skilled at masking autistic traits.

Masking may involve copying social behaviours, rehearsing conversations, suppressing natural movements, forcing eye contact, or constantly monitoring how one appears to other people.

Someone may therefore appear socially confident while privately experiencing exhaustion, anxiety, sensory overload, or the feeling that they are constantly performing.

For some adults, receiving an autism diagnosis or recognising autistic characteristics can provide an explanation for experiences they have struggled with since childhood.

ADHD: More Than Difficulty Concentrating

Attention-Deficit/Hyperactivity Disorder (ADHD) is another neurodevelopmental condition.

ADHD is often misunderstood as simply being distracted, hyperactive, or unable to concentrate. In reality, it can affect executive functioning, attention regulation, motivation, working memory, impulse control, emotional regulation, and perception of time.

Someone with ADHD may experience:

Difficulty starting or completing tasks
Forgetfulness
Frequently losing belongings
Impulsivity
Physical or internal restlessness
Difficulty organising and prioritising
Becoming easily distracted
Hyperfocus on highly engaging activities
Emotional overwhelm
Difficulty transitioning between activities
Time-management difficulties

ADHD does not necessarily mean someone cannot pay attention.

Often, the difficulty involves regulating where attention goes and how easily it can be redirected.

A person may struggle for an hour to begin a routine task while being capable of focusing intensely on an interesting activity for several hours.

These difficulties are not simply laziness or lack of discipline.

What Is AuDHD?

AuDHD is an informal term used within neurodivergent communities to describe someone who is both autistic and has ADHD.

It is not a separate medical diagnosis. Rather, an individual may meet the diagnostic criteria for both autism and ADHD.

Living with both can create an interesting and sometimes challenging combination of needs.

For example, an autistic person may strongly value routine and predictability, while their ADHD characteristics may create a desire for novelty and stimulation.

Someone may:

Want organisation but struggle to maintain it
Need routine while becoming bored by repetition
Seek sensory stimulation and also experience sensory overload
Desire social connection but become exhausted by interaction
Hyperfocus intensely before experiencing periods of reduced concentration

Understanding AuDHD can be particularly meaningful for people who have previously felt that ADHD or autism alone never fully explained their experience.

PTSD and Neurodivergence

Post-Traumatic Stress Disorder (PTSD) is a trauma-related condition rather than a neurodevelopmental condition.

PTSD can develop following exposure to traumatic events or experiences.

Symptoms may include:

Hypervigilance
Intrusive memories or nightmares
Avoidance
Emotional numbing
Feeling constantly unsafe
Heightened startle responses
Dissociation
Difficulty regulating emotions
Sleep disturbance

Some PTSD symptoms can overlap with autism or ADHD characteristics.

For example, concentration difficulties, emotional dysregulation, withdrawal, overwhelm, and heightened sensitivity may occur across different conditions.

However, the reasons behind those experiences can be very different.

It is also entirely possible for somebody to be both neurodivergent and traumatised.

One does not exclude the other.

Trauma-informed counselling should therefore consider both developmental history and life experiences rather than assuming every difficulty comes from one source.

OCD and Neurodivergence

Obsessive-Compulsive Disorder (OCD) is frequently misunderstood as simply being extremely tidy, particular, or organised.

OCD is much more complex.

It commonly involves obsessions — unwanted intrusive thoughts, images, sensations, or urges — and compulsions, which are behaviours or mental acts performed to reduce distress or prevent a feared outcome.

OCD may involve:

Repeated checking
Contamination fears or washing rituals
Counting
Reassurance seeking
Mental reviewing
Repeating particular behaviours
Distressing intrusive thoughts
Difficulty tolerating uncertainty

There can sometimes appear to be an overlap between OCD and autism, particularly around routines and repetitive behaviours.

However, the reason behind a behaviour matters.

An autistic routine may provide predictability, sensory regulation, comfort, or enjoyment. An OCD compulsion is typically associated with distress, intrusive thoughts, or the perceived need to prevent something feared.

Where symptoms are complex, professional assessment can help establish what is happening.

BPD, Emotional Regulation and Neurodivergence

Borderline Personality Disorder (BPD) is associated with significant difficulties involving emotional regulation, relationships, identity, and fears surrounding rejection or abandonment.

Someone with BPD may experience:

Intense emotional changes
Fear of abandonment
Difficult or unstable relationship patterns
Difficulty maintaining a stable sense of identity
Impulsivity
Feelings of emptiness
Intense reactions to perceived rejection
Self-destructive behaviours in some individuals

BPD remains a highly stigmatised diagnosis.

People living with BPD should not be reduced to labels such as “dramatic,” “attention seeking,” or “manipulative.” These stereotypes can increase shame and prevent people from accessing compassionate mental health support.

There can also be overlapping characteristics between BPD, autism, ADHD and trauma, particularly involving emotional dysregulation, relationship difficulties, rejection sensitivity, impulsivity, or identity.

These conditions are not interchangeable.

A thorough assessment should consider developmental history, relationships, trauma, sensory processing, communication patterns, emotional experiences, and the person’s wider circumstances.

Why Autism, ADHD, PTSD, OCD and BPD Can Sometimes Be Confused

Human experiences rarely fit perfectly into diagnostic boxes.

Similar symptoms can occur for very different reasons.

For example:

Difficulty concentrating may occur with ADHD, PTSD, anxiety, depression, sleep difficulties, or several factors simultaneously.

Emotional dysregulation can occur with ADHD, autism, PTSD, or BPD.

Repetitive behaviours may be associated with autism or OCD, but their underlying purpose may differ.

Social withdrawal might reflect autistic overwhelm, trauma, anxiety, depression, sensory overload, or exhaustion from masking.

This is why a list of symptoms found online cannot provide a reliable diagnosis.

Context matters. Developmental history matters. Trauma history matters. The individual matters.

Masking and the Impact of Being Misunderstood

One of the most difficult experiences for many neurodivergent people is spending years feeling misunderstood.

A child struggling with executive functioning may be described as lazy.

Someone experiencing sensory overload may be labelled difficult.

An autistic adult who has learned to mask effectively may repeatedly hear:

“But you don’t look autistic.”

Someone living with trauma may be criticised for being overly reactive.

A person with OCD may experience enormous shame about intrusive thoughts they never chose to have.

Someone with BPD may encounter stigma when they most need understanding and appropriate support.

Repeated misunderstanding can affect self-esteem, identity, relationships and mental health.

Labels should help us understand people. They should never become another way of dismissing them.

Neurodivergence and Relationships

Neurodivergence can also influence relationships and communication.

Differences in emotional processing, communication style, sensory needs, executive functioning, or the need for recovery time can sometimes create misunderstandings between partners, friends, colleagues, or family members.

Someone may need direct communication rather than hints.

Another person may need time alone after social interaction.

ADHD-related forgetfulness may be interpreted as not caring.

An autistic need for predictability may be misunderstood as controlling behaviour when it is actually related to regulation and reducing uncertainty.

Understanding the reason behind behaviour can transform communication and reduce unnecessary shame or conflict.

Neurodivergence-Informed and Trauma-Informed Counselling

Counselling should adapt to the individual rather than expecting every individual to fit the same therapeutic model.

Neurodivergence-informed counselling may involve:

Clear and direct communication
Respecting sensory needs
Allowing additional processing time
Understanding masking
Exploring executive functioning difficulties without judgement
Recognising overwhelm, shutdown, or burnout
Supporting emotional regulation
Working collaboratively

Where trauma is also present, counselling should be both neurodivergence-aware and trauma-informed.

The aim is not to make a neurodivergent person behave more neurotypically.

Therapy can instead provide a space to understand individual needs, reduce distress, process difficult experiences, strengthen boundaries, and develop strategies that work with the person’s brain and nervous system.

Moving From “What’s Wrong With Me?” to “What Do I Need?”

For someone who has spent years feeling different, inconsistent, difficult, or misunderstood, discovering neurodivergence can create an entirely new framework for self-understanding.

Likewise, recognising and receiving appropriate support for PTSD, OCD, BPD, or another mental health condition can open the door to treatment and recovery.

Understanding these experiences should not be about collecting labels.

It should be about replacing shame with understanding.

Instead of asking:

“Why can’t I cope like everybody else?”

we can begin asking:

“What does my brain and nervous system need?”

That can be a profound shift.

Final Thoughts

There is no single correct way to think, communicate, learn, regulate emotions, or experience the world.

Autism, ADHD and AuDHD can shape neurological development and everyday experience. PTSD, OCD and BPD are distinct conditions, but they can coexist with neurodivergence and sometimes create overlapping presentations.

Understanding those differences is important.

But understanding the person is even more important.

A diagnosis can provide answers, validation, appropriate treatment, and access to support, but it should never become the entirety of somebody’s identity.

With appropriate understanding, accommodations, therapeutic support, and self-compassion, people can begin moving away from years of self-blame and towards something far more valuable:

Knowing and understanding themselves.

Sometimes, finally understanding why you have experienced the world differently is where meaningful change begins.
